Known Contraindications

  • Warning
Hypoadrenocorticism, including acute Addisonian crisis (collapse), can develop at any dose; adrenal necrosis and sudden death are very rare. Check electrolytes and an ACTH stimulation test 4-6 hours after dosing at 10 days, 4 weeks, 12 weeks, then every 3 months and after every dose change. If the test is non-stimulatory, stop for 7 days and restart at a lower dose.
Sources: VMD SPC · VMD · FDA
  • Warning
Do not use with primary hepatic disease or renal insufficiency, in pregnant or lactating animals or animals intended for breeding. Extreme caution with anaemia; diabetes mellitus needs specific monitoring; wait at least a month after mitotane. Pancreatitis risk is raised in hyperadrenocorticism.
    Sources: VMD SPC · VMD
  • Warning
Hyperkalaemia risk with ACE inhibitors or potassium-sparing diuretics (spironolactone); deaths, including sudden death, have been reported with an ACE inhibitor. Use together only after a risk-benefit decision.
Sources: VMD SPC · VMD · FDA
  • Warning (Cat)
Cat (off-label): weight loss, urinary tract infection, chronic kidney disease, seizures, recurrent pancreatitis and hypocortisolaemia were seen during treatment. Stop and recheck if the cat stops eating or becomes lethargic.
