For urinary incontinence associated with urethral sphincter incompetence; efficacy is established only in ovariohysterectomised bitches. Determine weight accurately. Do not use with non-selective MAO inhibitors or during pregnancy/lactation. Use caution in cardiovascular disease, severe hepatic/renal disease, diabetes mellitus, hyperadrenocorticism, glaucoma, hyperthyroidism or other metabolic disease. Keep the child-resistant closure secured: accidental overdose is toxic and high doses may be fatal, especially in children.

vetman assessment
vetman’s 10-point assessment brings together a product’s regulatory credibility, clinical efficacy, safety and practicality. Cost-effectiveness is assessed only when reliable data are available. It is informational and does not replace a veterinarian’s clinical judgment for an individual patient.

Phenylpropanolamine 40.28 mg/ml syrup for dogs
Assessed clinical scopeDog · Complete authorised scope: oral treatment of urinary incontinence associated with urethral sphincter incompetence in bitches, with efficacy established only in ovariohysterectomised bitches · By mouth
Care settingLong-term owner-administered treatment after veterinary confirmation of a storage disorder, with an episode diary, baseline/periodic blood pressure and planned response/adverse-effect review
vetman score 8.0/10 Very good
Evidence confidence: High Georgia: Yes, veterinary · № 219-11; 352-11 · Verified on 23 Oct 2026

A very good, strongly supported first-line oral treatment for correctly diagnosed urethral-sphincter incontinence in spayed bitches. Its strong efficacy is balanced by TID adherence, blood-pressure and interaction monitoring, narrow diagnostic scope and a disclosed salt/base wording inconsistency in the label.

Regulatory and scientific credibility9.0/10

The exact product has current transparent Georgian and EU authorisations, a current regulator label and a public generic assessment bridging it to same-strength Propalin with essential similarity and favourable benefit-risk. The only material documentation weakness is no new exact-brand outcome programme, which a valid generic application did not require.

Clinical efficacy9.0/10

PPA is a unanimous ACVIM first-line option for female-dog USMI. The exact 1 mg/kg-equivalent TID reference regimen achieved complete continence at day 28 in 85.7% versus 33.3% with placebo, and independent FDA field evidence supports the active and endpoint. Benefit is strong but restricted to correctly diagnosed sphincter disease.

Safety and tolerability7.0/10

Most labelled adverse events are rare or very rare and fivefold overdose caused no effects in healthy-dog label data, but sympathomimetic hypertension, tachyarrhythmia, agitation, GI effects, proteinuria, collapse and seizures can be clinically important. Cardiovascular/metabolic comorbidity, multiple interactions and potentially fatal child ingestion require active controls.

Clinical practicality6.5/10

The supplied syringe, tiny weight-based volume, food administration, child-resistant cap and three-month in-use life help. Three-times-daily chronic dosing, accurate small-volume measurement, syringe washing/drying, episode tracking and periodic BP review remain a substantial owner and clinic adherence burden.

Important safety and use flags

  • Scope boundary: this score covers only urinary incontinence associated with urethral sphincter incompetence in bitches; efficacy is demonstrated only in ovariohysterectomised bitches. It does not cover behavioural urination, every male/intact dog or undifferentiated leakage.
  • Diagnose before treating: classify storage versus voiding disease and assess history, examination, urinalysis/culture and post-void residual or imaging as indicated. In bitches under 1 year investigate ectopic ureter or another anatomic disorder; also exclude infection, polyuria and neurologic disease.
  • Exact volume: after accurate weighing give 0.1 ml/5 kg PO three times daily on food. Do not substitute the ACVIM 2 mg/kg q8-12h or US tablet regimens for this product-specific syrup instruction.
  • Salt/base audit: the leaflet calls the dose 1 mg phenylpropanolamine/kg, but 0.02 ml/kg x 40.28 mg/ml equals 0.8056 mg/kg free base and x 50 mg/ml equals 1 mg/kg hydrochloride. Preserve the unambiguous labelled volume; do not generate a different volume from the ambiguous active name.
  • Measure systolic blood pressure before treatment and periodically. Reassess or stop for sustained hypertension, tachycardia/arrhythmia, marked restlessness/aggression, collapse, ataxia, seizure, significant vomiting/diarrhoea, appetite/weight loss, proteinuria or polyuria/polydipsia.
  • Use particular caution in cardiovascular disease, severe renal or hepatic insufficiency, diabetes mellitus, hyperadrenocorticism, glaucoma, hyperthyroidism or another metabolic disorder. Hypertension-prone patients may warrant an estrogen-first strategy or more frequent monitoring.
  • Do not use with non-selective monoamine oxidase inhibitors. Review other sympathomimetics, anticholinergics, tricyclic antidepressants and selective MAO-B inhibitors because cardiovascular/CNS effects can be amplified.
  • Do not use during pregnancy or lactation. No compatibility studies support mixing with another veterinary medicine. Hypersensitivity to phenylpropanolamine or an excipient is a contraindication.
  • Response and duration are individualized: record wet episodes and adverse effects, then reassess rather than issuing automatic indefinite repeats. Failure or relapse requires diagnosis/adherence review and a planned alternative or combination strategy—not unsupervised dose escalation.
  • Human safety is critical: replace the child-resistant cap immediately, never leave a filled syringe unattended and keep the bottle away from children. Overdose can cause hypertension, agitation, insomnia, nausea or headache and may be fatal, especially in children; accidental ingestion needs immediate medical advice. Wash and fully dry the dosing syringe; discard three months after opening.

Information not fully verified Exact authorised scope: urinary incontinence associated with urethral sphincter incompetence in bitches; efficacy is demonstrated only in ovariohysterectomised bitches. Give the exact labelled volume 0.1 ml/5 kg PO three times daily on food after accurate weighing. The composition is 40.28 mg/ml free base (=50 mg/ml hydrochloride), so this volume is 0.8056 mg/kg base or 1 mg/kg hydrochloride despite the leaflet wording “1 mg phenylpropanolamine/kg”; do not recalculate a different volume. Record wet episodes and reassess response, diagnosis and blood pressure.
