Pancreatic enzyme replacement is central to feline EPI care, but feline cohorts used unspecified powders and tablets and did not identify Mezym, coating, activity ratio or dose. No exact intact-tablet feline efficacy, dissolution, safety or dose study was found; cobalamin and concurrent disease materially affect response. A numerical exact-product score would therefore overstate the evidence.
The exact human product and its enzyme activities are well documented, and the veterinary therapeutic class is legitimate. The unresolved issue is product-level transfer: whole-only Mezym tablets cannot inherit efficacy or dose from powder, opened micropellet capsules, raw pancreas or unidentified products.
Important safety and use flags
- No exact intact-tablet dog/cat efficacy, dissolution, dose-ranging, repeated-use safety or clinical-outcome study was found. A veterinary class effect does not by itself create a Mezym product score.
- Mezym Forte 10 000 is a human medicine and is not authorised as a veterinary product. Georgian/EU human registration and current pharmacy availability do not establish veterinary benefit, safety or dose.
- Use only for confirmed exocrine pancreatic insufficiency. Nonspecific diarrhoea, weight loss, appetite change or dyspepsia requires diagnosis; this is not empirical digestive support or treatment for acute pancreatitis.
- Do not crush or chew the tablet. The exact label requires it to be swallowed whole because released enzymes can injure oral mucosa and chewing can reduce efficacy.
- Do not transfer the powder teaspoon regimen or the canine RCT regimen: the trial opened a different enteric-coated capsule and mixed its contents with food, and explicitly cautioned against extrapolation to other extracts.
- No dog/cat Mezym dose is published here. Human 2–4-tablet directions, lipase units, powder teaspoons and unspecified feline tablet use cannot be converted into one another without product-specific validation.
- Assess and correct cobalamin deficiency, especially in cats, and investigate concurrent intestinal, pancreatic, hepatic or endocrine disease when response is incomplete. Enzyme replacement alone may not resolve signs.
- Monitor body weight and condition, faecal consistency/frequency and clinical response; titrate only within a veterinarian-selected validated formulation. Poor response should trigger diagnostic and formulation review, not automatic Mezym escalation.
- Stop and reassess for oral injury, vomiting, diarrhoea, abdominal pain, constipation/obstruction or hypersensitivity. Human high-dose fibrosing-colonopathy and hyperuricosuria warnings are cautions, not target-animal incidence estimates.
- Pregnancy, lactation, breeding, juvenile use and long-term exact-product safety in dogs and cats are not established. Use requires a patient-specific benefit-risk decision and an alternative with a validated veterinary regimen should be preferred when available.
