A veterinary formulary regimen exists, but direct feline combination evidence is sparse and this exact Speybact registration is suspended for absent GMP evidence. Current lawful supply and batch quality are unresolved, so the exact presentation cannot receive a responsible clinical score.
The active combination and veterinary IV use are clinically interpretable, but the exact product remains unscoreable while its Georgian registration is suspended for absent GMP evidence and current batch quality is unresolved. Comparator quality cannot validate Speybact.
Important safety and use flags
- This is a human product. All dog/cat use is off-label and must follow veterinary antimicrobial governance.
- The Georgian registration is suspended because the register reports no GMP certificate. Retail availability does not mean active authorisation or verified current batch quality.
- Reserve for a serious proven or strongly suspected susceptible bacterial infection. Obtain samples before treatment when feasible, review culture/MIC and de-escalate to the narrowest effective agent.
- Antibiotic therapy does not replace drainage, debridement, foreign-body removal, surgery or other source control, and this product is not routine prophylaxis or minor-infection therapy.
- The 10-30 mg/kg dose is the total 2:1 ampicillin+sulbactam mixture. Do not interpret it as 10-30 mg/kg of ampicillin plus additional sulbactam.
- Direct critically ill dog PK shows variable target attainment. Do not assume Enterobacterales or resistant-organism coverage without susceptibility and patient-context review.
- Direct feline combination PK, outcome and exact-product evidence were not located. Cat efficacy and safety confidence are lower and extrapolation must be explicit.
- Prepare ex tempore, maintain asepsis and compatibility, and verify the current exact pack for post-reconstitution limits. The human IM label does not establish a companion-animal IM regimen.
- Avoid in serious beta-lactam hypersensitivity/cross-reactivity or prior ampicillin-sulbactam cholestatic injury. Adjust for renal impairment and consider sodium load, seizures at excessive exposure, GI/hepatic injury and cytopenias.
- Never reconstitute or mix with an aminoglycoside because ampicillin can inactivate it in vitro. Monitor clinical response, culture/MIC, renal/hepatic status, CBC, GI tolerance and bacterial/fungal superinfection.
