Tranexam

500 mg, film-coated tablet, human medicine
Assessed clinical scopeDog · Off-label oral tranexamic acid as continued adjunctive treatment for selected canine hyperfibrinolytic bleeding when an oral regimen is clinically appropriate · By mouth
Care settingVeterinary-directed outpatient continuation after the bleeding phenotype, thrombotic risk, renal function and an accurately measurable oral regimen have been assessed
vetman score 5.4/10 Weak evidence support
Evidence confidence: Medium Georgia: Yes, human · № რ-039091 · Verified on 16 Oct 2026 Off-label use

A weak but potentially useful oral adjunct for a selected canine hyperfibrinolytic bleeding phenotype. The ingredient has direct canine PK/PD data, but outcome benefit remains uncertain and this whole-only 500 mg form is a poor dose-delivery match outside a narrow weight range.

Regulatory and scientific credibility6.0/10

The exact human presentation has a sourced current Georgian registration and identifiable strength and manufacturer, but no exact trade-name registration was found in the exhaustively checked EU/EEA, UK or US registers. It has no veterinary authorisation and animal use is off-label.

Clinical efficacy5.0/10

Direct canine crossover data show oral absorption and reduced fibrinolysis in a laboratory model, but clinical outcome evidence is sparse and mixed. Trials of tranexamic acid in bleeding dogs have not consistently reduced transfusion, bleeding or survival outcomes, so oral use remains a selective adjunct rather than a proven routine therapy.

Safety and tolerability6.0/10

Short-course canine experience is usable but not benign: vomiting is dose- and rate-sensitive, and thrombosis, hypotension with rapid IV administration, renal accumulation, seizures and rare hypersensitivity require selection and monitoring. These exact human products have no canine target-animal safety dossier.

Clinical practicality4.0/10

A whole-only 500 mg tablet matches approximately 25-33 kg at the studied 15-20 mg/kg oral exposure, but is poorly measurable for many patients and the regimen may require dosing every 8 hours. Crushing does not validate an unmeasured powder fraction; use a measurable alternative for other doses.

Important safety and use flags

  • Do not use with active intravascular clotting or a current thromboembolic event. An antifibrinolytic can stabilize an unwanted thrombus as well as a haemostatic clot.
  • Tranexamic acid is not a universal haemostatic. Do not delay surgical or procedural source control, resuscitation, diagnostic coagulation assessment or indicated blood-component therapy while giving it.
  • Tranexamic acid is predominantly renally eliminated. Check renal function and hydration and reduce exposure or extend the interval when clearance is impaired; accumulation increases neurologic and thrombotic risk.
  • Use cautiously in patients with a seizure history or neurologic disease. Stop and reassess for myoclonus, twitching, altered mentation or seizures, particularly with high or accumulated exposure.
  • Concurrent prothrombotic products or a strongly hypercoagulable state may add thromboembolic risk. Document the rationale and monitor when combining with coagulation-factor concentrates or other haemostatic drugs.
  • Nausea and vomiting are clinically relevant and can be dose- or administration-rate related; protect the airway in obtunded patients and reassess repeated emesis, aspiration risk or inability to retain oral doses.
  • Stop immediately for suspected hypersensitivity, facial swelling, bronchospasm, collapse or anaphylaxis. A rare canine anaphylactic reaction has been reported and exact human products lack target-animal pharmacovigilance.
  • This exact 500 mg tablet is recorded as whole-only. Do not estimate a smaller dose from an unmeasured fraction of crushed powder; use an accurately measurable formulation when 500 mg is not the intended unit.
