vetman assessment
vetman’s 10-point assessment brings together a product’s regulatory credibility, clinical efficacy, safety and practicality. Cost-effectiveness is assessed only when reliable data are available. It is informational and does not replace a veterinarian’s clinical judgment for an individual patient.

Telartan

80 mg, tablet, human medicine
Assessed clinical scopeDog · Off-label oral telmisartan for persistent renal proteinuria in dogs, with blood-pressure benefit considered when hypertension coexists · By mouth
Care settingVeterinary-prescribed home treatment after persistent renal proteinuria and hydration status are confirmed, using an accurately deliverable dose with UPC, pressure, creatinine and potassium follow-up
vetman score 6.2/10 Situational / acceptable
Evidence confidence: Medium Georgia: Yes, human · № რ-036774 · Verified on 16 Oct 2026 Off-label use

Telmisartan has credible canine antiproteinuric evidence, but this 80 mg human tablet is off-label and its half-tablet step limits accurate dosing. Use only when the calculated dose is physically deliverable or prescribe a verified compounded liquid, with early renal, potassium, pressure and UPC review.

Regulatory and scientific credibility6.0/10

The exact human tablet is currently registered in Georgia with a sourced strength and manufacturer, but no current exact trade-name match exists in the checked EU/EEA, UK or US registers and no tablet is authorised for veterinary use. Veterinary telmisartan is transparently authorised in cats, but those liquid labels do not validate this tablet presentation.

Clinical efficacy8.0/10

A double-masked randomized trial in dogs with persistent renal proteinuria found a larger UPC reduction with telmisartan than enalapril through day 90; two additional clinical cohorts support antiproteinuric and blood-pressure effects. The trials used a liquid preparation, so equal tablet dose delivery must not be assumed.

Safety and tolerability6.5/10

Veterinary studies and labels define the main risks, but these exact human tablets have no species-specific safety dossier. Hypotension, dehydration-associated renal decline, azotaemia and hyperkalaemia require baseline selection and early reassessment; combining RAAS blockers can materially increase renal risk.

Clinical practicality3.0/10

The smallest recorded half-tablet is 40 mg, a very coarse 40 kg dose step at 1 mg/kg. Direct use is limited to selected very large dogs; most patients require a lower strength or verified compounded liquid.

Important safety and use flags

  • Do not start during dehydration, hypovolaemia, clinically important hypotension or acute kidney injury; stabilise the patient and establish persistent renal proteinuria first.
  • Do not routinely combine telmisartan with another ARB or an ACE inhibitor; dual RAAS blockade can cause hypotension, hyperkalaemia and clinically important renal decline.
  • Measure UPC from appropriately collected urine and assess blood pressure, creatinine/SDMA, potassium and hydration before treatment; repeat soon after starting or increasing the dose.
  • Review diuretics, NSAIDs and other medicines that lower pressure or renal perfusion; illness with poor intake, vomiting or diarrhoea can make temporary interruption necessary.
  • Reassess if creatinine rises materially, potassium increases, appetite or weight falls, weakness or syncope develops, or systolic pressure becomes too low.
  • Treat the underlying cause and use a renal diet where indicated; a lower UPC is a response marker, not proof that chronic kidney disease has stopped progressing.
  • The feline 1 mg/kg proteinuria regimen and the higher hypertension regimen are not interchangeable. Confirm the exact concentration and indication before calculating millilitres.
  • Reproductive safety is not established for veterinary use; the exact 4 mg/ml feline label contraindicates use during pregnancy and lactation.

Evidence confidence: Medium Georgia: Yes, human · № რ-036774 · Verified on 16 Oct 2026 Off-label use

Telmisartan is evidence-based in cats, but this 80 mg human tablet is a poor and potentially hazardous match to the small feline dose. Prefer the authorised 4 mg/ml veterinary solution or a validated patient-specific liquid and monitor renal function, potassium, pressure and UPC.

A 224-cat randomized blinded trial and current CKD consensus support telmisartan for persistent feline renal proteinuria, while a separate controlled trial supports antihypertensive efficacy. These data establish the active ingredient, not accurate dosing from these very high-strength human tablets.

Clinical practicality0.0/10

A half-tablet is 40 mg, far above a typical cat dose. This presentation is not directly doseable for cats and would require validated pharmacy compounding; the authorised feline liquid is markedly preferable.

Known Contraindications

  • Critical
Do not use during pregnancy or lactation.

  • Critical
Do not use during pregnancy or lactation.
    Sources: VMD SPC · VMD
  • Critical
Do not use in cases of hypersensitivity to the active substance or to any of the excipients.

  • Critical
Do not give a RAAS inhibitor to a cat that is clinically dehydrated or shows signs of hypovolaemia. Restore hydration first.
    Sources: IRIS
  • Warning
In cats with severe kidney disease, drugs acting on the RAAS can lower the glomerular filtration rate and worsen kidney function. The safety and efficacy of telmisartan in these cats have not been studied.

  • Warning (Cat)
The safety and efficacy of telmisartan has not been tested in cats under the age of 6 months.
    Sources: VMD SPC · VMD
  • Caution
Elevated liver enzymes... Values normalised within a few days following cessation of therapy.

Known Drug Interactions

  • Major warning Benazepril + Telmisartan
Available data in cats with CKD show no known interaction between telmisartan and other drugs acting on the RAAS (ARBs, ACE inhibitors), but combining drugs that act on the RAAS can change kidney function.
Recommendation: Use an ACE inhibitor and an angiotensin-receptor blocker together only after an individual benefit-risk assessment. Monitor creatinine, potassium, blood pressure, and hydration closely so deterioration in renal function is detected promptly.
Sources: VMD SPC · VMD · IRIS
  • Minor warning Telmisartan + Amlodipine
During concomitant therapy with amlodipine at the recommended dose, no clinical evidence of hypotension was observed.
Recommendation: Combine amlodipine and telmisartan only when either drug alone does not control blood pressure adequately. Measure blood pressure before treatment, correct dehydration, and recheck regularly after stabilisation.
