vetman assessment
vetman’s 10-point assessment brings together a product’s regulatory credibility, clinical efficacy, safety and practicality. Cost-effectiveness is assessed only when reliable data are available. It is informational and does not replace a veterinarian’s clinical judgment for an individual patient.

Telmitraxx

4 mg/ml, oral solution for cats
Assessed clinical scopeCat · Reduction of persistent renal proteinuria in an adequately hydrated, clinically stable cat with confirmed chronic kidney disease · By mouth
Care settingVeterinarian-directed chronic home treatment after CKD/proteinuria confirmation, with early and serial UPC, blood-pressure, renal-function and haematology monitoring
vetman score 8.4/10 Very good
Evidence confidence: High Georgia: No · Verified on 22 Oct 2026

A very good, label-concordant once-daily veterinary option for persistent renal proteinuria in a stable hydrated cat with CKD. Exact generic equivalence, a large controlled feline trial and current consensus support the score; the benefit claim remains proteinuria reduction, with mandatory renal, blood-pressure and haematology monitoring.

Regulatory and scientific credibility8.5/10

Exact current veterinary product identity, SPC and GB/EU authorisations are public, and regulators accepted Telmitraxx as a generic of Semintra 4 mg/ml. The product is not registered in Georgia or the United States exact endpoint; the former US NADA match belonged to 10 mg/ml Semintra.

Clinical efficacy8.5/10

A 224-cat blinded multicentre randomised trial found telmisartan 1 mg/kg once daily noninferior to benazepril and significantly reduced UPC from baseline; current IRIS and iCatCare guidance supports the same role and dose. Evidence supports proteinuria reduction, not proven CKD reversal or survival prolongation.

Safety and tolerability8.0/10

Exact feline safety material and long clinical experience support good tolerability under correct selection. Dehydration/hypovolaemia, hypotension, severe renal dysfunction, reduced erythrocyte indices, pregnancy/lactation, gastrointestinal effects and concurrent RAAS inhibition remain meaningful labelled or consensus monitoring limits.

Clinical practicality9.0/10

The ready once-daily oral liquid, exact 0.25 ml/kg conversion, supplied syringe, direct-or-small-food administration and six-month in-use period are highly workable for chronic feline care. Accurate weighing, complete dose ingestion and serial monitoring are still required.

Full-course cost-effectiveness-

No verified current Georgian pack price, availability or patient-adjusted long-term course cost was found; foreign prices were not substituted.

Important safety and use flags

  • This score covers only reduction of persistent renal proteinuria in a stable, adequately hydrated cat with confirmed CKD. It does not cover unselected CKD, acute kidney injury, nonspecific renal support, dogs or systemic hypertension.
  • Do not substitute Semintra 10 mg/ml or its hypertension regimen. US NADA 141-501 is a different 10 mg/ml product dosed initially twice daily; Telmitraxx is 4 mg/ml and this assessment uses 1 mg/kg = 0.25 ml/kg once daily for CKD proteinuria.
  • Confirm persistent renal proteinuria and address pre-renal/post-renal causes, urinary sediment/infection and hypertension before attributing protein loss to CKD. Borderline UPC 0.2–0.4 usually requires monitoring rather than automatic treatment; current guidance treats persistent UPC >0.4.
  • Do not initiate in a clinically dehydrated or hypovolaemic cat. Correct volume status first because RAAS inhibition can precipitate a marked fall in glomerular filtration; acute or rapidly changing kidney disease is outside this rating.
  • Measure blood pressure, creatinine/renal markers, electrolytes, UPC and haematology at baseline and early after initiation, then serially. A small stable creatinine rise can occur; marked or progressive azotaemia, hypotension, weakness, anorexia or dehydration requires prompt reassessment.
  • Do not combine with another ACE inhibitor or angiotensin-receptor blocker as routine therapy. Other antihypertensive or diuretic treatment can amplify hypotension or renal effects; document the combination rationale and monitor closely.
  • Do not use during pregnancy or lactation. Safety is not established in breeding cats or kittens under six months; pregnant people should avoid exposure to RAAS-active solutions and wash skin immediately after accidental contact.
  • Stop and reassess for persistent vomiting/regurgitation, diarrhoea, reduced appetite, weight loss, lethargy, hypersensitivity or liver-enzyme elevation. Reduced erythrocyte indices are a known class/product monitoring issue rather than a reason to omit baseline haematology.
  • Use the supplied syringe and current body weight. Give directly into the mouth or with only a small amount of food and confirm the whole dose is consumed; do not hide it in a full meal. Avoid contaminating the syringe or bottle opening.
  • Store below 30°C in the original container and use within six months after opening. Record the opening date. Georgian registration and current local availability are absent, so procurement and replacement continuity must be confirmed before starting long-term therapy.

  • Critical
Do not give a RAAS inhibitor to a cat that is clinically dehydrated or shows signs of hypovolaemia. Restore hydration first.
    Sources: IRIS
  • Warning
In cats with severe kidney disease, drugs acting on the RAAS can lower the glomerular filtration rate and worsen kidney function. The safety and efficacy of telmisartan in these cats have not been studied.

The safety and efficacy of telmisartan has not been tested in cats under the age of 6 months.

Known Drug Interactions

  • Major warning Benazepril + Telmisartan
Available data in cats with CKD show no known interaction between telmisartan and other drugs acting on the RAAS (ARBs, ACE inhibitors), but combining drugs that act on the RAAS can change kidney function.
Recommendation: Use an ACE inhibitor and an angiotensin-receptor blocker together only after an individual benefit-risk assessment. Monitor creatinine, potassium, blood pressure, and hydration closely so deterioration in renal function is detected promptly.
Sources: VMD SPC · VMD · IRIS
  • Minor warning Telmisartan + Amlodipine
During concomitant therapy with amlodipine at the recommended dose, no clinical evidence of hypotension was observed.
Recommendation: Combine amlodipine and telmisartan only when either drug alone does not control blood pressure adequately. Measure blood pressure before treatment, correct dehydration, and recheck regularly after stabilisation.
