Important safety and use flags
Do not administer to a patient with known cobalt, cobalamin or product-excipient hypersensitivity. Parenteral vitamin B12 has rarely caused anaphylaxis; have observation and emergency treatment available, especially after a previous unexplained injection reaction. Confirm deficiency with an appropriate cobalamin assay and, when results or clinical response are equivocal, methylmalonic acid or other diagnostic context. Do not use cyanocobalamin as an empirical tonic, nonspecific appetite stimulant or substitute for investigating weight loss, anaemia, neuropathy or chronic gastrointestinal signs. Treat the cause of deficiency, including chronic enteropathy, ileal disease, exocrine pancreatic insufficiency, altered intestinal anatomy or an inadequate diet. Cobalamin can correct a deficiency marker but cannot by itself control inflammation, infection, pancreatic failure, neoplasia or malnutrition. Recheck clinical response and serum cobalamin after the replacement course and consider methylmalonic acid when functional deficiency remains uncertain. Draw planned baseline samples before supplementation and interpret very high post-treatment concentrations in light of timing rather than escalating automatically.
