vetman assessment
vetman’s 10-point assessment brings together a product’s regulatory credibility, clinical efficacy, safety and practicality. Cost-effectiveness is assessed only when reliable data are available. It is informational and does not replace a veterinarian’s clinical judgment for an individual patient.

Metronidazole Belmedpreparaty

5 mg/ml, solution for infusion 100 ml, human medicine
Assessed clinical scopeDog · Temporary intravenous metronidazole for a hospitalised patient with a proven or strongly suspected susceptible obligate-anaerobic infection when enteral treatment is not feasible; not routine treatment of diarrhoea · Intravenous
Care settingHospital treatment with aseptic single-patient withdrawal, light protection, controlled infusion, neurologic monitoring and early conversion to a suitable oral veterinary presentation when possible
vetman score 6.5/10 Situational / acceptable
Evidence confidence: Medium Georgia: Yes, human · № რ-035616 · Verified on 16 Oct 2026 Off-label use

A credible hospital option for selected serious anaerobic infection when oral treatment is impossible, but not a default diarrhoea infusion. The score reflects precise 5 mg/mL dosing alongside off-label use, limited exact-product veterinary evidence and antimicrobial stewardship.

Regulatory and scientific credibility6.0/10

The exact human presentation has a sourced current Georgian registration and identifiable strength, route and manufacturer; exhaustive checked EU/EEA, UK and US registers contained no exact trade-name match. It is not veterinary-authorised.

Clinical efficacy6.5/10

Metronidazole has reliable activity and tissue penetration against susceptible obligate anaerobes, and human IV formulations have an established veterinary role in serious canine anaerobic infection. Outcome evidence for these exact bags is indirect and source control or broader therapy can remain essential.

Safety and tolerability6.0/10

Gastrointestinal adverse effects and marked microbiome disruption are relevant, and cumulative exposure can cause vestibulocerebellar neurotoxicity at lower doses than historically assumed. Reduce exposure in hepatic dysfunction and stop promptly for neurologic signs.

Clinical practicality8.0/10

The ready-to-use 5 mg/mL infusion permits accurate small-animal dosing in hospital, but the 100 mL single-use bag creates aseptic handling and wastage constraints; infusion compatibility and exact-product instructions must be verified.

Full-course cost-effectiveness-

Price was not assessed and is excluded from the overall score.

Important safety and use flags

  • Do not use metronidazole routinely for uncomplicated acute diarrhoea, haemorrhagic diarrhoea that responds to fluids, nonspecific dysbiosis or chronic enteropathy. Establish a defensible anaerobic/protozoal target and reassess the need for any antimicrobial.
  • Do not use after hypersensitivity to metronidazole, another nitroimidazole or an excipient. Stop and treat urgently for facial swelling, bronchospasm, collapse or anaphylaxis.
  • Stop metronidazole promptly for ataxia, nystagmus, tremor, seizure, altered mentation, weakness or peripheral neuropathy. Neurotoxicity can follow cumulative or prolonged exposure and has occurred in dogs at lower doses than historically assumed.
  • Severe hepatic dysfunction can reduce clearance and increase neurotoxicity. Adjust the regimen, avoid unnecessary duration and monitor clinical status; renal failure can permit metabolite accumulation.
  • Avoid unnecessary exposure during pregnancy or lactation and use only after an individual benefit-risk assessment. Human labels carry mutagenicity/carcinogenicity concerns, and feline in-vitro genotoxicity adds uncertainty rather than proving clinical cancer risk.
  • Review warfarin or other coumarins, phenobarbital, phenytoin, cimetidine, lithium, ciclosporin, busulfan and alcohol-containing preparations. Exposure or effect can change and targeted monitoring may be required.
  • Vomiting, nausea, anorexia, diarrhoea or hypersalivation can occur. Metronidazole can markedly disrupt the canine intestinal microbiome for weeks, so repeated empirical courses are not microbiota-sparing treatment.
  • This 5 mg/mL human infusion is a single-dose 100 mL container. Verify the exact product insert, solution integrity, light protection, infusion rate and compatibility; do not add unverified medicines, and discard unused solution rather than sharing a breached bag between patients.

Clinical efficacy6.0/10

Activity against susceptible obligate anaerobes and direct feline IV pharmacokinetics support a hospital role, but feline clinical-outcome evidence for human infusion bags is sparse; source control and broader therapy may remain essential.

Safety and tolerability5.5/10

Feline PK is documented, but direct safety data are limited; in-vitro genotoxicity and a severe neurotoxicosis case add uncertainty. Avoid unnecessary or prolonged exposure, account for hepatic disease and stop promptly for neurologic signs.

Clinical practicality8.0/10

The ready-to-use 5 mg/mL infusion permits accurate small-animal dosing in hospital, but the 100 mL single-use bag creates aseptic handling and wastage constraints; infusion compatibility and exact-product instructions must be verified.

Known Contraindications

  • Critical
Do not use in cases of hepatic disorders.
    Sources: VMD SPC · VMD
  • Critical
Do not use in cases of hypersensitivity to the active substance or to any of the excipients.
    Sources: VMD SPC · VMD
  • Critical
In dogs, high doses can cause neurotoxicity: tremor, muscle spasms, ataxia and even seizures, especially after a long course. The risk rises when the recommended dose or treatment length is exceeded. If neurological signs appear, stop metronidazole and treat the signs.

  • Warning
Studies in laboratory animals have shown inconsistent results with regard to teratogenic/embryotoxic effects of metronidazole. Therefore, use of this veterinary medicinal product during pregnancy is not recommended.
    Sources: VMD SPC · VMD
  • Warning
Metronidazole is excreted in milk and use during lactation is therefore not recommended.
    Sources: VMD SPC · VMD
  • Warning
tremors, muscle spasms, ataxia, and even convulsions
    Sources: MSD Vet Manual
  • Warning (Cat)
Metronidazole in cats: neurotoxicity (ataxia, altered mentation, tremors, seizures) is reported with high doses and long courses (58 mg/kg a day for 6 months; 111-222 mg/kg a day; 73.5-147 mg/kg a day). Keep to the label dose and duration; stop the drug at the first neurological sign.

Known Drug Interactions

  • Major warning Metronidazole + Phenobarbital
Phenobarbital may increase hepatic metabolism of metronidazole resulting in decreased serum concentration of metronidazole.
Recommendation: Metronidazole may be less effective than expected in a patient receiving phenobarbital. Monitor clinical response.
