  • Critical
Do not use trimethoprim/sulfonamide combinations when water intake is reduced or body-fluid loss is present. Maintain adequate hydration during treatment to reduce crystalluria and renal injury.
  • Warning
Safety during pregnancy and lactation has not been established. Long-term exposure produced fetotoxic effects in laboratory mice; use only after an individual benefit-risk assessment.
  • Warning
Safety during lactation has not been established. Use only after an individual benefit-risk assessment.
  • Warning (Dog)
Trimethoprim/sulfonamide treatment can cause keratoconjunctivitis sicca in dogs, particularly during courses longer than 7 days. Perform a baseline Schirmer tear test, repeat it periodically, and stop promptly if tear production falls or ocular discharge develops.

  • Warning (Dog)
Dogs are particularly susceptible to idiosyncratic sulfonamide reactions, including fever, blood dyscrasias, polyarthropathy, skin eruptions, acute hepatopathy, and proteinuria. A breed-associated risk has been proposed in Dobermans, but the evidence is uncertain.

  • Caution
During prolonged trimethoprim/sulfonamide treatment, perform periodic haematology and monitor thyroid function when clinically indicated. High doses can cause reversible hypothyroidism in dogs.

Known Drug Interactions

  • Minor warning Trimethoprim/Sulfonamide + Ganciclovir
Although systemic exposure after ophthalmic ganciclovir is low, an interaction with trimethoprim/sulfonamide combinations cannot be excluded; additive bone-marrow toxicity is possible.
Recommendation: Systemic absorption from the eye gel is low; perform haematology monitoring during prolonged combination therapy.
    Sources: Virgan SmPC
  • Minor warning Trimethoprim/Sulfonamide + Procaine Benzylpenicillin
Procaine (in procaine benzylpenicillin) is an ester of para-aminobenzoic acid (PABA). Like other PABA-ester local anaesthetics such as tetracaine, it can reduce the effect of potentiated sulfonamides, at least locally.
Recommendation: Account for a reduced sulfonamide effect during concomitant administration.
    Sources: MSD Vet Manual
  • Minor warning Amoxicillin + Trimethoprim/Sulfonamide
Concurrent use of bactericidal beta-lactam antibiotics and bacteriostatic antimicrobials such as sulfonamides is generally not recommended because their effects may be antagonistic.
    Recommendation: Avoid concomitant administration.
